When adding Task for Project from Report or Menu Tab, 'Add' button leads to no action. There is an uncaught error in the Java script or the script is being loaded out of sequence.

The firefox inspector has identified the source of the bug issuing a warning: "Unreachable code after return statement (firefox)"
There is an error in the return statement (main.js1), (chosen.jquery.js1)

All entities (tasks, tickets, etc), when enabled in the menu, fail to load with the errors previously reported. Please also see the attached.

for me this does not work at all, even if I removed all reports from any menu. Just on dashbord, add a new ticket >> select the project >> and then nothing
