Could this be an issue?
When I generate a DOCX report I always get this message.... I click Yes and then the report generates however it is all clumped up as shown previously.... When I click No then it does not generate anything of course.

I did test and it works ok with this source so first please check if you load files from archive in correct folder. Please do it manually and make sure all files replaced.
If problem still exist then please do more tests. Create new temp template for testing and use just description in template.
Try to print or save in pdf.
Let me know what you get in result.

Ok, thanks... still does not work... i must be doing something wrong.... I tried downloading as PDF and no text is being generated under the fields... see attached.... I will do some more testing and see...

Investigative Details is field in main entity where you create template or this is form subentity or related items?

It is subentity...
The way I structured it is as follow:
Cases (PRIMARY)
Case Reports (SUBENTITY FOR CASES) see attached 1
New Entry (SUBENTITY FOR CASE REPORTS) see attached 2
Then it has several fields (SEE ATTACHED 3-1, 3-2, 3-3)
The I created Template to Export (see attached 4, 5-1 and 5-2) for everything to be l=pulled into one DOCX report (see attached DOCX file)
Maybe because it is subentities causing this issue?

I think yes. Can you show me block settings for block with ID 96Maybe because it is subentities causing this issue?
I will try to reproduce template with the same settings.
