Entities - adding new entity (sub-item) results in database error / deletion not not possible

I've got 2 errors during creation of a new entity:

1. If a new entity is added to the entities (regardless of main or sub item) after selecting the SAVE button a database error occurs showing duplicate key name (see enclosed screenshot). After selecting the backward button in the browser the entity is showing properly in the entities list. It assumes, the new entity has been created properly.

2. If an entity should be deleted a warning occurs with message "You can't delete entity Test because it has sub items. Delete all items first, please.". Especially this occurs also for the newly created entity of topic 1. Going through the several tabs of the entity does not show any entry. Even the entity itself has no subitems.
